Limited Partnership Membership

Below are the documents your practice will need to review, complete and sign, and return in order to join CCPA Purchasing Partners, L.P. as a limited partner:

• CCPA Purchasing Partners Limited Partner Membership Checklist
• Limited Partnership Agreement
• Subscription Agreement
• W-9 (Request for Taxpayer ID Number and Certification)
• CCPA Purchasing Partners Compliance Committment Form (this form is optional but must be completed and returned in order to participate in our vaccine program)

Associate Membership

Below are the documents that your practice will need to review, complete and sign, and return in order to join CCPA Purchasing Partners as an associate member:

• Group Purchasing Participation Agreement for Associate Membership
• CCPA Purchasing Partners Compliance Committment Form (this form is optional but must be completed and returned in order to participate in our vaccine program)
