- Your child's medical records and primary care doctor's written referral.
- A list of any current medications, including the name of medicine, the dose and frequency.
- Your insurance information.
Before the visit

Contact your insurance providers, primary care physician and your primary oncologist or doctor currently managing your care to let them know you have an appointment with us. It may help to review our billing and payment policy.
