Prepare for your child's first appointment
Thank you for choosing us for your child's healthcare needs. Here are suggestions to help prepare, and ensure your child receives the best possible care.
Before the visit
- Need to cancel or reschedule your appointment? Please call 312.227.6090 between 8 a.m. and 4:30 p.m., at least 24 hours in advance. It helps us, and it helps other families receive timely care.
- Fill out this medical family history form, and bring it with you.
- Review our general first appointment information to help you prepare.
Bring with you
- Your child's previously prescribed medication.
- Any notes and/or test results from any other endocrinologist who has seen your child.
- Your child's medical record, including growth charts, all laboratory results and physician notes on the medical reason for your child's visit. Please fax to 312.227-9403 or mail to:
