Children's MyChart

Your First Cardiology Visit

Thank you for choosing us for your child's healthcare needs. Here are some suggestions to ensure your child receives the best possible care.

Seeing a pediatric cardiologist? Before your visit...

  • Need to cancel or reschedule an appointment? Please call us at 773.880.4553 at least 24 hours in advance. It helps us, and it helps other families receive timely care.

  • Plan to arrive at least 15 minutes before your scheduled appointment time.
  • Complete our cardiology questionnaire and bring it to your child's appointment.

  • Gather all the other necessary documents to bring, including:

    • Your child's previously prescribed medication.
    • Any notes and/or test results from any other cardiologist who has seen your child.
    • Your child's medical record. Please fax to 773.880.8111 or mail to:
      Division of Cardiology
      2300 Children's Plaza, Box 21
      Chicago, IL 60614-3363

  • Learn how Family Services helps you navigate your healthcare experience. You can also read more about our unique approach to family-centered care.

Additional cardiac testing...

  • If before your scheduled visit the cardiologist determines your child needs an echocardiogram (ECHO) and/or electrocardiogram (ECG), you may be asked to come in before your originally scheduled time.
  • An ECG takes about 20 minutes; an ECHO takes about an hour.
  • If your visit requires a referral for insurance, please make sure that referral also includes orders for an ECG and an ECHO.

During your cardiology visit...

  • Your child may be seen by a pediatric nurse practitioner or a fellow prior to your pediatric cardiologist.
  • Your child may need laboratory work completed. As a result, please be prepared to
spend at least three hours with us.

After your cardiology visit...
