Whether you're looking for health information, an appointment or registration assistance, or detailed physician profiles, our pediatric nurses can assist you in accessing all of Children's Memorial's services or help you find a doctor in your area. The nurses are available on the phone from 8 a.m. to 5 p.m. Monday through Friday.

Register online or download our printable form* to mail or fax prior to appointments you have already scheduled.
