Find Adult Health Care Providers

One of the hardest parts of transitioning may be finding the right doctors to care for you as an adult.

  • We suggest starting early − a year or two before the 18th birthday
  • Be sure to ask your current doctors about their transition policy
  • Talk to friends and others who have a similar condition or situation, including disease-specific organizations
  • You can also contact your current insurance provider for suggestions
  • Review the American Academy of Pediatrics directory of primary care doctors who care for adults & childrens with chronic illness

  • Prepare for Your First Visit

    • Typical things to do before a first appointment include:
      • Transfer your medical records to the new doctor's office
      • Bring a portable medical summary
      • Be prepared to ask and answer questions about medications, and more
